Why Does Physio Not Fix My Back Pain?

Let me start with something important.

If physio didn’t fix your back pain… it doesn’t mean physio doesn’t work.

And it definitely doesn’t mean your pain can’t be solved.

But it does mean one thing:

Something was missed.

I hear this story every day.

“I did exercises.” “I went for weeks.” “It helped a bit… but the pain came back.”

And that’s frustrating.

Because you did what you were told.

So what went wrong?

1. Physio Often Treats the Symptom, Not the Mechanism

Most back pain is treated with general protocols provided by

NHS and even generic private clinics:

Stretch. Strengthen. Move more.

And yes… these can help

But back pain is not one condition.

It can come from:

  • Disc loading and mechanical stress
  • Facet joint irritation
  • Hip dysfunction transferring load to the spine
  • Poor load distribution through the kinetic chain

If the real source isn’t identified, the treatment becomes generic.

And generic treatment leads to temporary relief… not resolution. So don’t just download an exercise list from the internet and expect that to heal your condition. It won’t work likewise your past experiences in generic clinics.

Sometimes the pain is not coming from ongoing tissue damage.

It’s coming from the nervous system.

This is called central sensitisation.

Your system becomes more reactive.

Signals are amplified.

So even small movements can feel painful.

If treatment focuses only on muscles and joints…

You miss the biggest part of the problem.

4. You Stopped When It Felt Better

This is common.

Pain reduces → you stop treatment → pain returns.

Why?

Because feeling better is not the same as being fixed.

Tissues need time to adapt.

Movement patterns need to change.

Stopping early often brings the problem back.

5. The Plan Was Not Specific Enough

Exercises are not the treatment.

They are tools.

The real treatment is precision.

Right load. Right direction. Right timing.

If those are off… even good exercises won’t work.
